All players selected to participate in the ODP season need to complete the following forms: ODP Code of Conduct Form and the ODP Player Information & Medical Release Form - both of which will be available when you register on-line or can also be downloaded from this website if you have trouble printing them out at the end of the registration process.
The Player Profile Form is only needed for the 1995 and 1996 age groups and the Medical History Questionnaire will be needed by all players and will be available in the on-line acceptance registration link for all players who make the state pool and can also be downloaded from this website if you have trouble printing them out from the registration link.
All forms can be downloaded by clicking on the Forms link located to the left side of the home page under ODP Forms. All forms can also be downloaded in both doc and pdf form from the region1.com website.
Forms should be mailed (regular mail only) to WVODP c/o Tina Mascaro 108 Gilbob Street Fairmont, WV 26554 or brought to the required training event. Email odp@wvsoccer.net with any questions regarding these forms.
NOTE: IF YOU COMPLETE THE REQUIRED ON-LINE REGISTRATION THESE FORMS WILL BECOME AVAILABLE DURING THAT PROCESS AND YOU WILL NOT NEED TO DOWNLOAD THEM FROM THIS SITE.
