Instructions for Applying for Foreign Travel
1. Contact your state office
2. Request necessary state forms
3. Request US Soccer required forms

4. Complete the state forms along with the US Soccer forms
5. See back of 4 part application form to determine required US Soccer documents
US Soccer Application
Signed Ted Stevens Amateur Sports Act
Copy of official brochure, pamphlet, invitation or other applicable material of the tournament or games host
Copy of official roster of applying team which is in good standing, which includes all players and team officials, signed and/or stamped by appropriate registrar
Fee : $ 25.00 (made payable to US Soccer) Please note your application date determines if late fees apply…Late fees begin to apply if application is received at US Soccer less than 90 days prior to the travel date (See back of 4 part application form)…late fees, if applicable, must be included with payment
6. Submit completed forms to your state association office for their review and state certification of US Soccer forms
7.State office or applicant submits certified forms directly to US Soccer
